Submit your child's immunization records

A screen recording of Pearspark doing exactly this in a sample school, and the same 6 steps written out below in the labels you'll see on screen.

How to submit your child's immunization records

  1. 1On Home, click "Health" on your child's card.
  2. 2In the "Immunizations" card, click "Add shots / upload card" (schools that only keep the form on file show "Upload immunization form" instead).
  3. 3The form opens on "Immunization — list the doses": fill "Which vaccine?" (a dropdown when the school publishes a vaccine checklist), "Dose number" (blank if unsure) and "Date given"; click "Add another dose" for more rows.
  4. 4Attach a photo or PDF of the card from your doctor (required the first time; optional once a record is on file).
  5. 5Type your full name under "Type your full name to sign" and click "Send to the office".
  6. 6The dose shows under "Updates you've sent" as waiting for the health office until the office reviews it; the Immunizations card keeps flagging any required doses the school's records still don't show.
