How to complete the annual health information confirmation

1Click Home in the portal navigation bar and open your child's card
2Click the amber banner "Please review & confirm [child]'s health information for [year] →"
3Review the four cards: Allergies & conditions, Medications at school, Emergency information, and Immunizations
4If everything is right, click "Everything is accurate — confirm"
5Type your full name in the "Type your full name to sign" field and click "Sign & confirm"
6A green "Confirmed for [year]" badge appears at the top of the page
